Liability Waiver

World Famous Mixed Martial Arts

By participating in any program, class, training session, or activity at World Famous MMA, you acknowledge and agree to the following:

Assumption of Risk
Mixed martial arts, fitness training, and related activities involve physical exertion and inherent risks, including but not limited to injury, strain, falls, collisions, and other unforeseen incidents. You voluntarily assume all risks associated with participation.

Release of Liability
You release and hold harmless World Famous MMA, its owners, instructors, staff, and affiliates from any and all claims, damages, injuries, or losses arising from participation in any activity, whether supervised or unsupervised.

Medical Responsibility
You certify that you are physically capable of participating and have consulted a physician if necessary. You agree to notify staff of any medical conditions that may affect your safety.

Emergency Care
In the event of an emergency, you authorize World Famous MMA staff to seek medical assistance on your behalf. You are responsible for any resulting costs.

Acknowledgment
By signing up for membership or participating in any activity, you acknowledge that you have read, understood, and agree to this Liability Waiver.
